If you’re coping with a chronic illness or high-risk pregnancy, our award-winning health management programs can help. They can provide education and support to help you understand and participate more confidently in the management of your condition. Our programs focus on conditions such as asthma, chronic obstructive pulmonary disease (COPD), diabetes, heart failure, coronary artery disease, and high risk pregnancy (only applies to plans with maternity coverage). Each program is available free of charge to members who have one or more of these conditions.
We were one of the first health plans in the nation to receive a full, three-year accreditation from the NCQA for all of our health management programs.
